Position Title: Licensed Pharmacist

Locations: 757 45th Street, Suite 102, Munster, IN 46321

Anticipated Start Date: As soon as possible

Expected Duration: Full Time

Job Type: Full Time - Direct Hire

Shift Hours: Monday–Friday, 8:00 AM – 4:30 PM

Salary Range: 145,600.00/Annually

• Preferred Skills/Experience: Previous leadership and specialty pharmacy experience (preferred over retail pharmacy experience).

• Essential Functions

Medical Team Collaboration (5%)

• Collaborates with allied health disciplines to provide quality patient care.
• Provides clinical consultation and clarification concerning drug therapy, medication reconciliation, patient care, and transitions of care.
• Attends rounds, patient care meetings, or similar activities as warranted.

Staff and Patient Consultation (5%)

• Consults with and advises healthcare professionals and patients on the selection, procurement, distribution, and safe and appropriate use of medications.

Staff Education (5%)

• Coordinates and delivers ongoing education for clinical staff pharmacists.
• Ensures professional competence of pharmacy staff through continuous educational opportunities.

Student and Resident Supervision (5%)

• Directs and precepts pharmacy students and/or residents, providing mentorship and training.

Formulary Management (5%)

• Leads cost-effective formulary management initiatives.
• Develops, reviews, and implements formulary protocols and policies.
• Recommends appropriate, cost-effective therapeutic alternatives to the medical staff.

Medication Safety & Process Improvement (5%)

• Reviews adverse events related to the medication use process and recommends improvements.
• Ensures identified improvements are implemented effectively.
• Reviews current literature and publications to recommend process enhancements.

Medication Order Review (5%)

• Reviews prescriptions and medication orders for appropriateness.
• Provides necessary information to ensure proper and safe administration.
• Identifies and corrects inappropriate orders promptly, documenting interventions.
• Reports adverse drug reactions and medication errors in the event reporting system.

Local and System Collaboration (5%)

• Participates in local and system-wide committees focused on improving the medication use process.

Medication Policies (5%)

• Drafts, reviews, and revises medication-related policies, procedures, guidelines, and protocols.

Clinical Pharmacotherapy Services (5%)

• Provides clinical pharmacy services as outlined in hospital policies, including but not limited to:
• Pharmacokinetic consults
• Renal dosing adjustments
• Parenteral nutrition management
• Anticoagulation management
• Neonatal/pediatric/geriatric dosing optimization
• Route optimization
• Antimicrobial stewardship
• Medication reviews during transitions of care
